Based on observation, the licensee did not comply with the section cited above, Licensee has not conducted a fire drill i nthe last 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will conduct a fire drill and log the fire drill, Licensee will send copy of log to LPA Sims via email to sydney.sims@dss.ca.gov
Based on record review, the licensee did not comply with the section cited above in 5 of 6 counts of not having LIC 627 in children's files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain LIC 627 for C1 - C4 and C6 and put form in children's files. Licensee will send copies of LIC 627 for C1 - C4 and C6 to LPA Sims by 8/16/24 via email to sydney.sims@dss.ca.gov
