Based on observation the licensee did not comply with the section cited above in infant (C6) sleeping on the couch with a blanket which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee and staff will review safe sleep video provided by LPA Sims, Licensee and staff will provide statement stating that Licensee and staff will comply with safe sleep regulations to LPA SIms by 12/8/25.
Based on observation and record review, the licensee did not comply with the section cited above, Licensee failed to report which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have fire department come to facility and clear addition to home. Licensee will inform LPA Sims when fire clearance has been given. Licensee will update facility sketch to include addition and submit to LPA Sims by 1/2/25
