Based on record review, the licensee did not comply with the section cited above in one count of staff S2 not having a file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will create a staff file for staff S2. LPA provided entrance checklist LIC 126 to Licensee for reference. Licensee will send copy of staff file to LPA Sims via email to sydney.sims@dss.ca.gov by 02/12/26
Based on record review, the licensee did not comply with the section cited above, no staff at facility have current CPR or first aid training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send proof of completion of CPR / First aid class or proof of enrollment in a CPR first aid class to LPA Sims by 02/12/26 via email to sydney.sims@dss.ca.gov
Based on record review, the licensee did not comply with the section cited above, Licensee is not conducting 15 minute check for any infants in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will watch safe sleep video provided by LPA SIms, and send statement to LPA Sims stating that Licensee will agreed to follow the safe sleep regulations. Licensee will also conduct one weeks worth of 15 minute checks for every infant enrolled ( 5 infants enrolled) at facility and send copy of the 15 minute safe sleep checks to LPA Sims via email to sydney.sims@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above, in five counts of the Licensee not having immunozations on file for children C2 - C6 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain immunizations for children C2 - C6 and provide copy of immunizations to LPA Sims via email to sydney.sims@dss.ca.gov by 02/12/26
Based on observation, the licensee did not comply with the section cited above in two counts of C6 sleeping in a rocker and C7 sleeping a swing which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct 1 weeks worth of 15 minute checks and send copy of logs to LPA Sims by 06/06/25 via email to sydney.sims@dss.ca.gov.

Based on observation and interview, the licensee did not comply with the section cited by admitting to caring for more children than the license allows and by LPA observation of the licensee caring for 11 children alone which exceeds the capacity of the small family child care home license. This presents an immediate risk to children in care.
Resolution: The licensee will submit a written plan to the Department stating how to remain in compliance with capacity/ratio requirement to ensure no future citations occur to the no later than close of business on 9/26/2023.
Based on observation, interview and record review, the licensee did not comply with the section cited by admitting to not having a current/completed roster and by LPA observation of an outdated roster with missing information. This presents a potential risk to children in care.
Resolution: The licensee will submit a copy of an updated and completed roster to include the names and contact information or all children in care by 10/9/2023.
Based on observation and interview, the licensee did not comply with the section cited by admitting to allowing infants to sleep with loose object and articles in their cribs at nap time and by LPA observation of 2 infants with a pillow, blanket, and stuffed animal in their cribs while sleeping. This presents an immediate risk to children in care.
Resolution: The licensee will watch the safe sleep video on how to properly put infants to sleep, review safe sleep regulations and submit a written plan to the Department no later than close on business on 9/26/2023.
