Based on observation, the licensee did not comply with the section cited above in 2 counts, Licensee had two infants sleeping with blankets in the pack n play which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will review safe sleep regulations provided by LPA Sims and write statement agreeing to follow all safe sleep regulations by 12/9/24 and have all parents with children currently enrolled sign LIC 9224 by next buissness day or next day child is enrolled. Licensee will have all children who enroll for the next 12 months sign Lic 9224.
Based on observation, the licensee did not comply with the section cited above, Licensee was unable to provide carbon monoxide detector during inspection. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will get a carbon monoxide detector and provide proof of installed carbon monoxide detector to LPA Sims by 01/03/25
Based on observation, the licensee did not comply with the section cited above, Licensee supervising 8 children with out an assistant and no children were in kindergarten or 6 years olld which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will review ratio requirments and sent statement agreeing to follow the ratio requirments to LPA sims by 01/03/2025
Based on record review, the licensee did not comply with the section cited above in Licensee and assistant have expired mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Assistant will complete mandated reporter training at mandatedreporterca.com by 1/1/24 and send copy of certificate to LPA Sims via email to sydney.sims@dss.ca.gov
