Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee Padmini Srinivasan was unable to provide proof of their mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof of their AB1207 mandated reporter certification by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews at 8:20 a.m., LPA observed child #1 (C1) did not have immunization records inside their file. LPA asked licensee for the immunization records and licensee stated the records were not available during the visit which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will submit immunization records for C1 to LPA by POC due date
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed C1 did not have an infant safe sleep log and licensees could not provide proof showing they were documenting safe sleep checks for C1. LPA also observed child #2 (C2) has not had a safe sleep log since 5/22/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit infant safe sleep logs from 12/18/2023 through 12/22/2023 for all infants by poc due date.
