Based on interview and record review, the licensee did not comply with the section cited above in one out of six reviewed staff files. Staff #3 (S3) did not have current Mandated Reporter training. When asked, the Assistant Director stated although S3 was hired since 05/2025, S3 has not submitted the certificate to the facility as of 09/17/2025, which posed a potential health, safety or personal rights risk to persons in care.
