LPA observed child #2 (C2) and child #3 (C3) did not have proof of immunization records inside their file and immunization records were not available for C2 and C3 during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit documentation for child #2 (C2) and child #3 (C3 by poc due date
