Director Madison Sanello was notified to cease enrollment of the children under the age of 2 years old. The plan of correction was due by 3/10/2025. LPA observed the Notice of Site Visit from the inspection conducted on 3/7/2025 posted on the parent board. Today, LPA observed 1 child under the age of 2 years old napping with 1 staff. The facility has failed to correct the deficiency issued on 3/7/2025 for section code 101161 Limitations of Capacity by the 3/10/2025 due date. This is an immediate health and safety risk to the children in care. This is a type A violation under Health & Safety Code 1596.99. The facility is being issued a civil penalty for $100/per day due to the a failure to correct the plan of correction that was due on 3/10/2025. LPA observed the Notice of Site Visit from the inspection conducted on 3/7/2025 posted on the parent board. Today, LPA observed 1 child under the age of 2 years old napping with 1 staff. The facility has failed to correct the deficiency issued on 3/7/2025 for section code 101161 Limitations of Capacity by the 3/10/2025 due date. This is an immediate health and safety risk to the children in care. This is a type A violation under Health & Safety Code 1596.99. The facility is being issued a civil penalty for $100/per day due to the a failure to correct the plan of correction that was due on 3/10/2025. LPA reviewed children’s files and did not observe the LIC9224 (Acknowledgment of Receipt of Licensing Reports). Director stated that the LIC809 Facility Evaluation Report dated 3/7/2025 was not given out to the parents since the decision was under appeal.
