Based on record review and facility representatives own admission, the facility did not comply with the section cited above. Facility representative was only able to provide documentation of 15 minute sleep log for 1 day, 07/25/25. LPAs informed facility representative that the 15 minute sleep checks need to be documented and kept in the infants' file for the duration the child(ren) is/are an infant. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility agrees to immediately implement and keep record of 15 minute sleep logs for all infants who are attending and currently or newly enrolled. Documentation must be kept on file for the duration child(ren) is/are an infant. Proof of logs are to be submitted to the department no later than POC due date.
Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed staff member did not have a current Mandated Reporter Training certificate.
Resolution: Licensee stated he would provide a current certifiacte and send to LPA by 5/7/24.
