Based on observation, interview and record review, the licensee did not comply with the section cited above in that Licensee was not observed to be present in the home and was out due to personal matters. It was confirmed by Facility Representative Tiancheng "Charles" Xu that Licensee does not reside at 9360 Ledig Dr, Rancho Cucamonga, CA 91701, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written a plan of action on how licensee will maintain compliance with above regulation and submit proof of Plan of Correction (POC) to Community Care Licensing (CCL) on or before POC due date.
