Based on record review, the licensee did not comply with the section cited above. Director stated they have not done a disaster drill yet, and was unable to provide proof of one done in the last 6 months. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, record review and directors own admission, the licensee did not comply with the section cited above. There was no proof of written approval and instructions from the child's authorized representative for the administration of the medication to the child. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observations, the licensee did not comply with the section cited above. The kitchen has 2 cabinets that hold hazardous chemicals with no lock. The kitchen door always remains open, for ventilation purposes, children do access the area near the kitchen door. Furthermore, there is a drawer with kitchen knives also accessible. This poses/posed a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents. This was not met as evidenced by: During a tour of the center, LPA noted areas of the center not being used with signs of rodents. One out of the two pieces has completely worn down, exposing sharp pieces of wood. The water fountain itself works, and has collected dirt and leaves on it, as well as calcium from water. This poses/posed a potential health, safety or personal rights risk to persons in care.

S1, S3 and S4 did not have MRT on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
