Based on observation, the licensee did not comply with the section cited above, poisons, detergents, and cleaning compounds are in the garage (off limits area) and the interior garage door is not key locked, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof that poisons and toxins located in the garage have been key locked to LPA via email by COB on 02/01/2024.

Based on record review, the licensee did not comply with the section cited above, documentation of the last fire/disaster drill conducted is not completed, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document the last fire/disaster drill that occurred within the last six months and provide proof to the LPA via email by COB on 02/15/2024.

Based on observation, the licensee did not comply with the section cited above, the fishpond lower gate is self-closing, swings away from the pond but is not self-latching. The upper gate is not self-closing, does not swing away from the pond and not self-latching, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to repair the fishpond lower gate so that it is self-latching, the upper gate is self-closing, swings away from the pond and is self-latching and provide proof to the LPA via email by COB 02/15/2024.

Based on record review. the licensee did not comply with the section cited above, C1, C2, C3, C4, and C5 are missing form LIC9150, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and obtain parent signatures of form LIC9150 for C1, C2, C3, C4, and C5 and provide proof to LPA via email by COB on 02/15/2024.

Based on record review, the licensee did not comply with the section cited above, C1 and C3 are missing infant sleep log documenting 15-minute sleep check, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct 15 minute sleep check and document infant sleep log for C1 and C3 and provide proof to LPA via email by COB 02/15/2024.
