LPA and ss met with Director, Nicole Ramirez, toured the facility, and took census. The Unusual Incident Report (UIR) was received by the licensing agency on 10/30/2025. It indicated a child was left in the classroom by themselves, resulting in an absence of supervision Records were reviewed, and pertinent parties were interviewed. Based on the information gathered, although the facility acted appropriately with reporting, there was still a violation of Title 22 Regulations. Furthermore, the following violations were also identified during the inspection: based on record review, S1 only had Mandated Reporter Training (MRT) on file for "general" and not "child care providers". S1 was missing proof of Tdap vaccination. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.
