Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure mandated reporter training was completed for all staff by the expiration date which poses a potential safety risk to children in care.

Resolution: LPA discussed the mandated reporter training requirement and Licensee was reminded to ensure all staff renew the course every two years. Licensee agreed to provide completed certificates for themselves and S1 to LPA by 02/26/2026.

Based on observation, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure Affidavit Regarding Liability Insurance for Family Child Care Home (LIC 282) forms were completed and maintained for four out of five children present which poses a potential safety risk to children in care.

Resolution: LPA discussed the requirement to maintain a file of affidavit for each child enrolled. Licensee agreed to provide copies of completed LIC 282 forms for C1, C2, C3, and C5 to LPA by 02/26/2026.

Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA's observed the licensee did not have a pool alarm or pool cover

Resolution: Licensee sated she will send a video of the pool alarm sounding and send to CCL by 4/3/25
