Based on the interview/record review, the Licensee did not meet Operation of a Family Child Care Homes which poses an potential Health, Safety & Personal Rights risk to the children in care. LPA reviewed the fire drill log observed the last fire drill was conducted on 1-28-2021. During the interview the Licensee stated a fire was conducted in July, however, she forgot to document it.

Resolution: Licensee agrees to complete and submit proof of fire drill to the Department on or by the POC due date.

Based on the interview/record review, the Licensee did not meet Mandated Reporter which poses an potential Health, Safety & Personal Rights risk to the children in care. LPA reviewed Mandated Reporter certificates for all staff that expired in August 2021. During the interview the Licensee stated there are no current Mandated Reporter certificates available for review.

Resolution: Licensee agrees to complete Mandated Reporter training and submit certificates to the Department on or by POC due date.

Based on the interview/record review, the Licensee did not meet Safe Sleep which poses an potential Health, Safety & Personal Rights risk to the children in care. LPA reviewed children's files and discovered C9 did not have an LIC 9227 available for review. During the interview the Licensee stated there is not an LIC 9227 available.

Resolution: Licensee agrees to submit an LIC 9227 for C9 to the Department on or by the POC due date

Based on the interview/record review, the Licensee did not meet Safe Sleep which poses an potential Health, Safety & Personal Rights risk to the children in care. LPA reviewed children's files and discovered C7, C8, C9 and C10 did not have sleep logs available for review. During the interview the Licensee stated there are no sleep logs available.

Resolution: Licensee agrees to submit a sleep log for C7, C8, C9, and C10. Licensee also agrees to ensure all children under the age of 24 months who enroll after today will have a sleep log on file.
