Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure their mandated reporter training was completed by the expiration date which poses a potential safety risk to children in care.

Resolution: LPA discussed the mandated reporter training requirement and Licensee was reminded to renew the course every two years. Licensee agreed to provide completed certificate to LPA by 03/20/2026.

Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure 15-minute sleeping logs were completed for three out of three infants which poses a potential health and safety risk to children in care.

Resolution: LPA discussed the requirement to check and log infant sleeping every 15 minutes for infants under 24 months. Licensee agreed to provide proof of completed 15-minute sleeping logs for C1, C3, and C5 to LPA by 03/20/2026.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed during record review 4 children's records did not have updated immunizations records.

Resolution: Licensee stated she will provide the updated immunizations to CCL by 1/16/24.

Based on record review, the licensee did not comply with the section cited above in persons which poses an immediate health, safety or personal rights risk to persons in care. LPA observed during record review adult working at the facility was not fingerprint cleared.

Resolution: Licensee stated she will have employee finish the documents required by Guardian and send proof to CCL by 11/12/24. Licensee will not have adult working in the facility until the adult is cleared.
