Based on observation, interview, record review, the licensee did not comply with the section cited above, Licensee's adult resident was not fingerprinted prior to residence in the facility, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated the adult resident shall have fingerprints taken by plan of correction date and email LPA LIVE SCAN receipt. LPA provided contact information and LIVE SCAN forms.
Based on observation, the licensee did not comply with the section cited above, Licensee does not have a barricade for stairs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall barricade stairs by plan of correction date. LPA shall return for a plan of correction inspection
Based on interview, and record review, the licensee did not comply with the section cited above, Licnesee Alejandro's certificate has expired, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Alejandro shall take Mandated Reporter training at www.mandatedreporterca.com by the plan of correction date and email the certificate of completion to LPA. LPA provided contact informaiton.
Based on oobservation, the licensee did not comply with the section cited above due to there not being a gate barricating the stairs. LPA Barragan obtaining photos of this, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above due to the Licensee not having a sleep plan, which poses a potential health, and safety risk to persons in care.
Resolution: The Licensee stated that she will ensure she gets a sleep plan for all children by the end of today (06/07/23.)

Resolution: The Licensee stated that she will ensure she starts documenting infant names and sleeping date/times every 15 minutes.
