Based on records review, staff S1 and Licensee was unable to present LPA with current Mandated Reporter certificates. Licensee was also unable to find S1's file and only had her negative TB screening on record. S1 is fingerprint cleared and associated but is missing her immunizations, current FA/CPR certificate, LIC 9108, and LIC 9052 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will provided proof of all missing personnel records/certificates to the department via email (patrick.ma@dss.ca.gov) by POC date.
Based on observation the garage, living room, and kitchen were missing barriers and mechanisms prohibiting access to those unlicensed areas when LPA first arrived for inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: During inspection, Licensee installed gate making living room and kitchen inaccessible. Licensee stated she will provided proof garage is inaccessible to the department via email (patrick.ma@dss.ca.gov) by POC date. Prior to correction, License will place furniture infront of the door.
