Resolution: LPA reviewed Safe Sleep regulations with licensee. LPA provided copies of sample 15 minute logs. Licensee will provide proof of the 15 minute logs for infants in care to the Department by POC due date. Licensee will review Safe Sleep regulations and provide a well written statement on what they have learned and submit it to the department by the POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed various items in the kitchen drawer and the restroom as stated in the LIC809 report, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's Assistant ensured to make items inaccessible to children in care during the inspection, by clearing items out.

Based on observation and the licensee did not comply with the section cited above, upon arrival LPA observed 13 children in care without an assistant present, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will make sure she does not go over capacity and will have her assistant present. She will have parents stick to agreed schedule of drop off and pick up times.

Based on record review, the licensee did not comply with the section cited above. Licensee could not provide proof of 15-minute sleeping log for 4 out of 4 infants present. Licensee stated she has never completed a sleeping log and did not know she needed to complete one for infants. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided licensee with sample of 15-minute sleep log today and reviewed safe sleep regulations. Licensee indicated she would complete infant sleep log for infants #1, infant #2, infant #3 and infant #4 and send a picture to LPA by POC due date 5/1/2024

Based on observation and record review, the licensee did not comply with the section cited above, upon arrival LPA Soto observed 4 out of 13 chlildren present were infants, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection one child was picked up. Licensee stated she will have to tell the parent of newly enrolled child that she can no longer have him enrolled in her daycare.

Based on observation, the licensee did not comply with the section cited above by having knives made accessible in a kitchen drawer without a child safety lock, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately moved knives to the top kitchen cabinet and stated she will keep them in top cabinet moving forward. If a child safety lock is installed licensee stated she whill send LPA a picture by 5/1/2024.

Based on record review, the licensee did not comply with the section cited above in 4 out of 7 children's files reviewed were missing immunization record which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will obtain proof of immunizations for child #1, child #2, child #3 and child #4 and maintain proof within the family child care home by 5/1/2024. Licensee will send a picture of children's missing immunizations record by 5/1/2024.
