Based on record review, the licensee did not comply with the section cited above. The facility was unable to produce proof of current mandated reporter training for Staff #1, Staff #2, and Staff #3 which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed that the blankets for all of the children were wrapped up together and stored on top of the napping cots. The children were also observed sleeping on the napping cots without sheets which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed several toys scattered on the floor near two trashcans in the dining room. There were supplies stacked in totes with missing lids leaning against a storage rack. The restroom floors were sticky and debris under the changing table pad. A play kitchen contained an unknown substance in the sink and a soiled rag laying on top. LPA observed other play houses that were covered in dust and contained bird droppings/ feces which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. As of 02/27/2025, per Guardian Employee Roster, Staff #1 is fingerprint cleared but not associated to Saint Anne's Preschool which poses a potential health, safety or personal rights risk to persons in care.

LPA also observed ZEP cleaning solution and a wash rag hanging from the sink located in the dining room which poses an immediate health, safety or personal rights risk to persons in care.
