Licensee could not provide proof of 15 minute sleep log and stated she does not complete it consistently for the infants in care. Resolution: LPA provided licensee with sample 15-minute log today and reviewed safe sleep regulations.Licensee indicated she would complete infant sleep log for infant in care by POC date 4/19/2024.

Based on observation, the licensee did not comply with the section cited above. Today, LPA Ocegueda observed a row of cinderblocks that were in stacks of two in the accessible yard. The cinderblocks were heavy and could potentially fall on children's feet creating a potential hazard. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would remove the cinderblocks to an inacessible area and provide proof to the Department by POC date 5/5/2023.

Based on record review and interview, the licensee did not comply with the section cited above. Licensee stated that S2 had not been able to complete required Mandated Reporter Training due to technical problems. Mandated Reporter Training was expired as of January 2022 for S2. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would have S2 complete Mandated Reporter Training. Licensee indicated that she was connected to her Resource and Referral agency and would seek assistance with this requirement if she still could not access the training on line.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee could not provide proof of 15 minute nap log and stated she does not complete it consistently for the infants (infant #3 and infant #4) in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided licensee with sample nap log today and reviewed safe sleep regulations. Licensee indicated she would complete infant nap log for infants #3 and infant #4 by POC date 5/5/2023.

Based on observation the licensee did not comply with the section cited above. Upon arrival, it was observed that one infant over the age of one year old was sleeping with two stuffed animals in the play yard. Licensee removed the items today and LPA discussed safe sleep requirements. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee was observed to remove the stuffed animals and stated she understood the regulation stated above. Licensee completed a written statement indicating understanding of the requirement as well. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above. LPAs Ocegueda and Zamudio reviewed children's files and observed that three out of five children's files were missing proof of required immunizations. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would submit proof of required immunizations for children #2, #4, #5 to the Department by POC date 5/23/2022.
