Based on record review and interview, the licensee did not comply with the section cited above. Upon file review, it was confirmed that child #6 was missing Identification and Emergency Information Form and did not have Consent for Emergency Medical Treatment (LIC 627) form. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would have the parent complete the required forms and all other licensing forms and documentation that were required by the Department. Proof of Identification and Emergency Information (LIC 700) and LIC 627 for child #6 will be submitted to the Department by POC date: 12/1/2023.

Based on record review and interview, the licensee did not comply with the section cited above. Child #6 did not have a nap log documenting every 15 minutes. This which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she would start to document nap log for infant #6 immediately and submit proof of infant nap log to the Department by POC date 12/1/2023.

Based on observation, the licensee did not comply with the section cited above. LPA Ocegueda observed that the kitchen walls had multiple smudge stains, splatter stains and looked generally unclean. Other surfaces in the kitchen also appeared unclean (stove, and counters) with excess smudges and old food particles. There were multiple wall outlets that did not have safety covers in the kitchen and living room. There were two car seats in the living room today which were accessible to infants and children. This poses a potential risk to the health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the carseats from the living room and placed in off limit rooms. Licensee placed plastic covers on the all the wall outlets. Licensee cleaned the surfaces of her kitchen stove and counters. Licensee stated she would be cleaning the walls in her kitchen and hallway and would be painting the wall in her kitchen to ensure that it was clean. Licensee was reminded that her home should be clean, orderly and have healthful and safe conditions for the children in care. Proof of clean or painted walls will be submitted to the Department by POC date 12/1/2023.

Based on record review, the licensee did not comply with the section cited above. Licensee's CPR/First Aid training expired in June/2022. Licensee showed she was enrolled in a course this week. This poses and posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee showed proof that she is enrolled in a CPR course for this week. LPA reviewed the regulation requirement with licensee. Licensee stated that in the future she will ensure that she is enrolled in a class before her CPR expires. Licensee will send proof of updated and current CPR training by POC date 12/27/2022.
