Based on observation, the licensee did not comply with the section cited above. LPA observed cleaning compounds (easy off and bathroom cleaner) that were accessible to the children under the kitchen sink which poses a potential health, safety or personal rights risk to persons in care.

Resolution: During the insepction licensee immediately removed the cleaning compounds from under the kitchen sink and made them inaccessible.

Based on observation the licensee did not comply with the section cited above. LPA observed approximately two large bottles of hand sanitizer on the bottom of a bathroom sink. There were approximately 8 bottles inside a bathroom sink drawer. In the accessible kitchen, there were multiple prescription medications and vitamins on the counter. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Today, Licensee removed the hand sanitizer bottles and placed them in inacessible areas. Licensee also removed medications and vitamins from the kitchen counter. Licensee stated she would complete a written statement indicating how she plans on keeping hazerdous items off limits to day care children. Proof of written statement will be provided by POC date : 11/17/2023.

Based on record review, the licensee did not comply with the section cited above. Today, infant #6 did not have Individual Infant Sleep Plan LIC 9227. This poses potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would submit a copy of Individual Infant Sleep plan for child #6 by POC date 12/1/2023.

Based on observation and interview, the licensee did not comply with the section cited above. It was observed that the kitchen counters had multiple objects, food products, pots, pans and dishes. It was difficult for LPA to inspect the counters due to the excess clutter and food debris. In the day care room, LPA observed that the two pack and plays were stained and dirty. Licensee stated that she had not washed them. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Today, licensee cleared the counters. Licensee stated she would clear her table and the rest of her counters to reduce clutter. Licensee stated that she would wash the pack and plays to remove stains. Proof will be submitted by POC date : 12/1/2023.

Based on record review, the licensee did not comply with the section cited above. Child #2, #3, and child #5 did not have Emergency Medical Consent form (LIC 627). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would have the parents complete this form for the children (#2, #3, #5) and place in their files. A return inspection will be conducted to verify the deficiency was corrected.

Based on observation, the licensee did not comply with the section cited above. LPA observed several cleaning products (Lysol, Clorox brand ect) under an accessible kitchen sink. On a kitchen counter, there was bleach. In an accessible bathroom drawer, there was toothpaste and sunscreen. The children also had access to power tools as the door in the living room the children were using was not locked or had child safety cover and the child gate that is typically in place was down today due to floor installation that was taking place. This a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that the floor installation would be completed today. Today, Licensee placed the baby gate back up making the power tools inaccessible. Today, Licensee placed a functioning safety device on the kitchen sink and removed the bleach. Licensee placed the kitchen knives on top of the refrigerator. Licensee removed the toothpaste and the sunscreen from the kitchen bathroom and made it inaccessible. Deficiency cleared today.

Based on observation and interview, the licensee did not comply with the section cited above. Today, LPA did not observe a play yard or crib for infant #2 and licensee indicated that she did not have a play yard or crib for infant #2. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would obtain a proper crib or play yard for infant #2 and understands that infants under 12 months old should only sleep in a crib or a play pen so long as they cannot climb out. A return inspection will be made to verify that the deficiency stated above is corrected.

Based on observation the licensee did not comply with the section cited above. An infant over the age of 12 months was observed sleeping in a high chair. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the infant out of the highchair today and placed her on a sleeping mat. Licensee will obtain safe sleep training via the following website: safetosleep.nichd.nih.gov. Licensee stated she will print out the transcripts of each of the four videos and sign and date them. A return inspection will be conducted to verify the deficiency was corrected.
