Resolution: Today, licensee stated her and her husband/assistant would complete Mandated Reporter Training and provide proof of certificate by POC date 11/22/2023.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed that there was toys tied on top of an infant play yard. There was an infant laying awake inside. This which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Today, licensee removed the toys. LPA advised of the regulation stated above. Deficiency cleared.

Based on observation, the licensee did not comply with the section cited above. LPA observed infant #1 sleeping in a play pen with a blanket underneath him/her. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA discussed safe sleep regulation requirements with licensee. Licensee removed the blanket and stated she would use tight fitted sheets or nothing at all in the play pens while children napped. Licensee completed a written statement indicating she understood that blankets would not be used during nap time. Deficiency cleared today.

Based on record review and interview, the licensee did not comply with the section cited above. S3 did not have completed Mandated Reporter Training as required. Per licensee, S3 has never completed this training. S3 was present today while S2 took a child to school and per licensee occassionally assists. This which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that S3 will complete the required Mandated Reporter Training and submit proof to the Department by POC date 10/21/2022. LPA provided licensee a list of all requirements that should be in facility files.

Based on observation the licensee did not comply with the section cited above. Infant #1 was observed sleeping on recalled equipment (Fisher Price Rock and Play sleeper). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed recall information with licensee and reviewed the requirement for infants to sleep only in a crib or play pen. Licensee removed the child and placed in play pen. The rocker was removed and placed outside where children dont have access to it. Licensee completed a statement indicating that she would not use this item for children at any time. Deficiency cleared today.

Based on observation the licensee did not comply with the section cited above. Today, LPA observed that licensee had two wrong sized fire extinguishers (1-A-10-BC instead of the 2-A-10-BC). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would purchase the right size 2-A-10-BC fire extinguishers and submit proof to the Department by POC date 10/21/2022.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee could not provide written proof that she had completed fire drills in the last 18 months. During interview, licensee confirmed that the last time she did a fire drill was in March of 2021. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would conduct a fire drill, document the fire drill and submit proof to the Department by POC date 10/21/2022. Licensee understands that fire drills should be conducted and documented every 6 months.

Based on record review, the licensee did not comply with the section cited above for 4 infants in care. Licensee admitted to not documenting sleeping checks for infants every 15 minutes,.which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to start to document 15-minute sleeping checks for children #1, 2, 3, 4 effective today. Deficiency cleared at time of visit.
