Based on observation, the licensee did not comply with the section cited above. LPA observed clorox cleaning product on the hallway bathroom floor which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed clorox cleaning product and placed it under the kitchen sink which is made inaccessible by a child safety latch.

Based on observation and record review, the licensee did not comply with the section cited above 1 out of 3 staff members did not have immunization record on file for pertusis, measles and TB which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit a copy of S3 immunzation record to the Department by POC date 9/30/2025.

Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 3 staff members did not have mandated reporter training on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated S2 will complete required Mandated Reporter Training and submit proof to the Department by POC date 9/30/2025.

Based on observation and interview the licensee did not comply with the section cited above. Today, LPA's Ocegueda and Pa Kou Vue observed Raid brand poison in the accessible kitchen sink. Outside in the off-limits yard that was accessible today were also weed killer and mosquito off spray. There were also various cleaners under the accessible kitchen sink that read "keep out of reach of children". Licensee confirmed the observation today and indicated she forgot to secure the door to the off-limit back yard. This poses an immediate risk to the health, safety and/or personal rights risk to persons in care.

Resolution: Today, licensee removed the Raid spray and cleaning supplies and placed in her locked vehicle. The other two poisons outside were placed in an inaccessible garage. Licensee stated she would gather all poisons and place them under key lock in a cabinet and make other cleaning solutions and potentially hazardous items inaccessible. Proof of locked poisons and inacessible cleaning supplies will be submitted to the Department by POC date 7/28/2023.

Based on observation the licensee did not comply with the section cited above. Upon inspection of the restroom outside, it was observed that the toilet area had some urine stains, the floors and base boards were dirty and the sink and sink fixtures were also unkept and stained. In the living room there were multiple shoes on the ground creating tripping hazards as well as papers and other objects on the sofa couch. This is a potential health, safety or personal rights risk to persons in care.

Resolution: Today, Licensee cleaned and sanitized the restroom as well as cleaned the living room, removing shoes and objects from the couch. LPA reminded licensee of the regulation stated above. Licensee agreed to monitor her home for clean and safe conditions at all times. Deficiency cleared today.

Based on file review, the licensee did not comply with the section cited above. Licensee and S3 could not provide proof of required immunizations (pertussis and measles) during the file review. Licensee and S3 also did not have proof of Tuberculosis. Licensee did not have staff documents available for Department review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would find proof of required immunizations and organize staff files with all required documents. LPA provided a list of documents that should be kept and available for Department review at all times. Proof of required immunizations will be submitted to the Department by POC date 8/25/2022.

Based on file review, the licensee did not comply with the section cited above. Licensee had expired Mandated Reporter training (expired on 8/2021) and S2 and S3 had not completed Mandated Reporter Training. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she and S2 and S3 would complete required Mandated Reporter Training and submit proof to the Department by POC date 8/25/2022.
