Based on record review the licensee did not comply with the section cited above. Licensee, S2 and S3 could not provide proof of required immunizations (Measles, Pertussis and flu). This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would submit proof of required documentation for the above named immunizations to the Department by POC date 5/20/2022.
Based on record review the licensee did not comply with the section cited above. Child #4 did not have emergency information or identification form. Licensee stated that parent had not submitted any of child #4's enrollment forms to her. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she would have parent of child #4 complete emergency information form along with all other required documentation and place in file of child #4. Proof of the required emergency information form and all other required forms/documents will be submitted to the Department by POC date 5/20/2022. A list of required forms was provided to licensee.
