Based on observation, interview, and record review], the licensee did not comply with the section cited above in 1 count out of 1 object which poses/posed a potential health, safety or personal rights risk to persons in care. Fire extinguisher is missing receipt and/ or service receipt from fire department.
Resolution: Licensee will purchase the a new fire extinguisher and save the receipt or have the current fire extinguisher serviced by the fire department for continued use. Licensee will notify and send proof of purchase or service to LPA with a picture of the receipt or documenation.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 count out of 1 objects which poses/posed a potential health, safety or personal rights risk to persons in care. The pool is missing a life ring, pool alarm, and daily inspection log.
Resolution: Licensee will purchase a pool alarm and a pool ring and send a picture the purchased items to LPA. Licensee will also complete the daily inspection log for the pool. Once all items are complete, licensee will send picture proof of the pool alarm in the pool, a life ring within the pool area, and daily inspection log being conducted.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 count out of 1 file which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee has not renewed their mandated reporter training. Certificate has expired on 8/3/2025.
Resolution: Licensee will renew the mandated reporter training and send picture of complete training to LPA.
