The incident occurred on 10/1/25 and was reported to licensing on 10/6/26. LPA spoke with lead teacher Arlene Franco about child's follow up and emergency treatment. A census was taken and 16 children are in care with 3 staff and two parents in the classroom. Staff are meeting state required regulations and child ratios. LPA reviewed sign in and out sheets verifying full signatures for 16 children in care. Teacher states the Great Shake Out earthquake drill will be conducted today at 10:20am. LPA took a self guided tour of the playground where child was injured. During inspection of the playground LPA observed all climbing equipment and poles to be appropriate for childrens use. Supervision was discussed and per teacher active supervision was provided during the child's injury. Proper authorities were contacted on 10/1/25 when child was injured they include fire truck ,paramedics, parents ,principle of the school and the school nurse. Per Teacher parent declined emergency services at the time of injury. Child did not attend school the following day , office staff contact parent to follow up with a medical clearance for child to return to school . Child was seen by medical professional Nicholle Henley D,O. at Pomona Valley Health Center Urgent care. He was cleared to return to school and clearance was provided to teachers and is on file in the classroom. Child is in class today and LPA observed child to still have very light small bump on forehead.
