Based on observation the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee she will provide the completed form to parent today and email back to LPA before POC date

Based on observation, the licensee did not comply with the section cited above in high chair trays that children eat directly off of is not clean, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to wash the highchairs and send LPA a photo by phone.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 6 children's files were not available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she has given parents documents but they have not returned it. License states she will speak to parents and obtain documents by the end of winter break. Licensee will submit photos of completed documents via phone by 1/10/25.

Based on observation, the licensee did not comply with the section cited above in licensee had 13 children in care while alone, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA observed 4 children leave family child care home during visit and assistant arrive to family child care home after transportation.

Based on observation, the licensee did not comply with the section cited above in cleaning compounds were accessible in the on limits kitchen to children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to purchase a child safety lock for the cabinet, install it and send a photo to LPA's cell by 12/19/24.

Based on record review the licensee did not comply with the section cited above. The Licensee does not have a current roster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with Roster Form LIC 9040. Licensee will complete roster and send LPA a copy.

Based on record review and Licensee's statement, the licensee did not comply with the section cited above. The Licensee's does not have children's files at hand. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will send in copies of children's records.

Based on LPA observation, the licensee did not comply with the section cited above. The home is visibly not clean or orderly in all areas of the home that children use. Licensee acknowledged this. This poses/posed a potential health, safety or personal rights risk to persons in care.
