Resolution: The licensee agrees to immediately conduct, document, and submit a copy of the fire drill conducted with children in care. The plan of correction shall be submitted to CCLD on or before 2/20/2026.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to renew the Mandated Reporter Training. A copy of her current certification shall be submitted to CCLD on or before 2/20/2026.

Based on record review, the licensee did not comply with the section cited above in 1 out of 5 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide a completed copy of Child #3's Notification of Parents' Rights to CCLD on or before 2/20/2026.

Based on observation, the licensee did not comply with the section cited above in 1 out object which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of removing cleaning componds, repairing or replacing the latches on the cabinet doors located under the kitchen sink. The plan of correction shall be submitted to CCLD on or before 9/20/2024.

Based on a record review, the licensee did not comply with the section cited above in one out of nine children's file reviews which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to submit a completed Notification of Parent's Rights (LIC995A) for C5 to CCLD on or before 11/15/2023.

Based on an interview, the licensee did not comply with the section cited above. The licensee stated a drill was completed in the summer, however, did not doument the drill as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide a copy of her doucmentation of an emergency disaster drill. The plan of correction shall be submitted to CCLD on or before 11/15/2023.

Based on LPA's observation, the licensee did not comply with the section cited above. Upon arrival to the home LPA observed the licensee caring to 9 children without an assistant present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to view the video provided on the Departments website (https://ccld.childcarevideos.org) regarding staffing and ratios. Once viewed the licensee will submit a written statement on how she will maintain staffing capacity ratios as required. The written statement shall be submitted to CCLD on or before 11/15/2023.

Based on children's record reviews, the licensee did not comply with the section cited above in 2 out of 9 file reviews which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide copies of Affidavit Regarding Liability Insurance (LIC 282) for C5 and C6. The plan of correction shall be submitted to CCLD on or before 11/15/2023.

Based on an interview, the licensee did not comply with the section cited above. The licensee's Mandated Reporter Training has expired and her assistant (A1) certificate was not available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to complete and have her assistant complete the MRT as required. The licensee agrees to provide proof of completion to CCLD on or before 11/15/2023.

Based on record review, the licensee did not comply with the section cited above in the licensee stated that she was unable to locate her updated roster of children in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide a copy of her current Roster of Children in Care. The licensee agrees to submit the plan of correction to CCLD on or before 11/15/2023.

Based on a record review, the licensee did not comply with the section cited above in one out of one record review indicating that her assistant (A1) proof of measles vaccine was not available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide a copy of A1 proof of measles vaccine. The plan of correction shall be submitted to CCLD on or before 11/1/52023.
