Based on interview where the licensee stated that Adult #3 did not obtain a California clearance as required after turning the age of 18, the licensee did not comply with the section cited above in 1 out of 3 persons which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to provide proof of Adult #3 getting livescanned. The plan of correction shall be submitted to CCLD on or before 1/13/2026.
Based on record review, the licensee did not comply with the section cited above in mandated reporter certificate expired on 2/24/24 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to take mandated reporter training and send LPA copy of certificate by 6/4/24. Send certificate to tammy.dutra@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above in CPR/First Aid Certificate was not EMSA certified which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to enroll in EMSA certified CPR/First Aid training and send proof of enrollment to LPA by 6/4/24. Send documentation to tammy.dutra@dss.ca.gov.
