Based on observation and record review, the licensee did not comply with the section cited above by, not consistently documenting 15 minute safe sleep checks while infant is sleeping which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and staff will watched Safe sleep video provided by LPA Sims from the CCL website and write statement acknowledging they watched it. Licensee will also document two days worth of safe sleep logs and send copy of log to LPA Sims via email to sydney.sims@dss.ca.gov
