Based on record review, the licensee did not comply with the section cited above in three out of three children's files were missing 15 minute sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to send several days worth of sleep checks for three infants in care. Send to email to CCL at tammy,dutra@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above in three infant files missing individual safe sleep plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to send three individual infant safe sleep plan on file for three infants in care. Send to CCL at tammy.dutra@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above. Assistant was missing immunization records and TB clearance, which poses a potential health, safety or personal rights risk to persons in care. Assistant states she is immunized and has been cleared for TB but does not have her documentation available right now.
Resolution: Licensee agrees to obtain proof of vaccinations and TB clearance for assistant and provide proof to CCL by 12/16/22.
