Resolution: Licensee agrees to get an updated plan to immunize C2 per health and safety code. C2 needs 2 each of Polio, DTP, Hib, Hep B to be complaint with regulations. Licensee will submit plan to LPA by 5/8/25. Send plan to LPA at tammy.dutra@dss.ca.gov.
