Based on observation, the licensee did not comply with the section cited above in 2 out of 2 play yards had non tight fitted sheets which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees will place tight fitted sheets on the (2) play yards and submit photo to LPA via email sarah.garcia@dss.ca.gov by 5pom on 8/22/25
Based on observation, the licensee did not comply with the section cited above in LPA observed an infant in a play yard with a blanket (loose article) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA discussed safe sleep regulations with licensees and licensees will sign a declaration stating they understand safe sleep regualtions and no loose arricles and objsects shall be in crivbs or play yards. Licensees will submit declarations to LPA via email sarah.garcia@dss.ca.gov by 5pm on 8/22/2025.
Based on interview and record review, the licensee did not comply with the section cited above. LPA reviewed 3 Infant files and did not see 15 minute safe sleep log. Licensee stated she does not have 15 minute safe sleep log. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA by 02/28/2024 3 days of 15 minute Infant safe sleep log for children in care that are 24 months and younger.
Based on observation, interview and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will sumbit current Mandated Reporter Training Certificate for both licesee and assistant through email or text message by due date
