Based on obsrevation and interview statements, the licensee did not comply with the section cited above in one out of four adults present in the home, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she would have Antonia Acevedo complete a LiveScan by tomorrow 2/21/24 and submit a copy to the LPA.

Based on records review and interview, the licensee did not comply with the section cited above in one of one infants enrolled, which poses an immediate health, safety or personal rights risk to persons in care. The licensee did not have documentation to show the 15 minute safe sleep checks are done as required.

Resolution: The licensee stated she will complete the logs and provide proof of correction.

Based on observation, the licensee did not comply with the section cited above in one of one child present, which poses/posed a potential health, safety or personal rights risk to persons in care. C3 was showing signs of illness while playing with the other children in care. .

Resolution: The licensee will isolate children who are showing sings of illness to prevent communicable disease. The licensee will send a declaration stating she understand the regulation.

Based on records review, the licensee did not comply with the section cited above in three of three staff present, which poses/posed a potential health, safety or personal rights risk to persons in care. Three staff present did not have a current mandated reporter in file.

Resolution: The licensee stated staff will complete the mandated reporter and provide proof of completion to the LPA

Based on observation, record review, 3 out 3 staff members mandated reporter training certificate were expired. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee state, they will complete and renew the mandated reporter training online at www.mandatedreporterca.com and submit a copy to licensing department by the due date.

Based on observation, interview, record review, during inspection at 9:00am LPA Odom observed 1 infant under 12 months, LPA reviewed child#1 file and napping log was missing for C1. Licensee stated they supervise infant while napping and inform parent, but a napping log was not created. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will document every 15 minutes when infant is napping. LPA provided a napping log sample. Licensee will submit document to licensing department by the due date.

Resolution: Licensee stated, they will provide a copy of the LIC9227 to C1's parents to complete and submit a copy of the completed form to licensing department by the due date.
