Based on record review, the licensee did not comply with the section cited above in that Staff # 2 did not have proof of a current mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee's son they will have staff # 2 complete the training and certificate will be submitted to LPA by POC due date of 12/08/25.
Based on observation and record review, the licensee did not comply with the section cited above in that Licensee and 2 assistants did not have proof of current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will all take mandated reporter training and submit copies of current certificate to LPA by POC due date of 10/18/2024. Training can be taken at mandatedreporterca.com
