Based on observation and record review, the licensee did not comply with the section cited above in LPA observed child 9 (C9), C10, and C11 did not have a file for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of completed files to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed licensee's assistant alone with 11 children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will ensure that ratios are met and will submit a declaration to LPA by POC due date. Licensee states that she is always in ratio; however, the second assistant had to leave early for the day.

Based on observation, the licensee did not comply with the section cited above in LPA observed the fire extinguisher was last serviced on 09/22/23, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will service the fire extinguisher and will submit proof to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed that the licensee is missing proof of the mandated reporter training AB1207. Licensee only took the general training which poses posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will take the mandated reporter training AB1207 and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed 9 out of 14 children were listed on the roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will update the children's roster and will submit proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above in that assistant does not have a completed staff file for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee they will have assistant complete all forms for their employee file and will have it available for department review.

Based on interview and record review, the licensee did not comply with the section cited above in that licensee did not have a sleep log available for 3 out 3 infants enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will start documenting the infant's sleep next time the infants are present and will submit copies to LPA by POC due date of 8/2/2024.

Based on record review, the licensee did not comply with the section cited above in that assistant does not have current mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have assistant take training and submit copy of certificate to LPA by POC due date of 8/26/2024.

Based on records review and interviws, the licensee did not comply with the section cited above in Licensee and Assistant do not have current Mandated Reporter training.which poses a potential health, safety or personal rights risk to persons in care.
