Based on observation, , the licensee did not comply with the section cited above in that the required 2A10BC Fire Extinguisher was last serviced on 7/18/2025 as indicated on the service tag which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will service the fire extinguisher and be available for department review by POC due date of 1/30/2026
Based on observation, interview and record review, the licensee did not comply with the section cited above in that child # 2 is missing a complete file, including emergency form which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will ask parents to complete all forms needed for child # 2- documents will be available for department review by 1/30/2026
Based on observation, the licensee did not comply with the section cited above in LPA observed (3) infants had crib sheets hanging on the side of their play pen which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee and co-licensee immediately removed all crib sheets. LPA provided printed PIN 20-24-CCP-SP and PIN 19-02-CCP for review. Licensee and co-licensee will review PINs and will submit a written declaration stating what they learned from both PINs by POC due date. LPA also provided a copy of LIC 855.
Based on observation, the licensee did not comply with the section cited above in LPA observed (1) infant in their play pen awake with a pink beaded string attached to the pacifier. It is unknown if the infant had recently woken up from nap which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the infant from the play pen since they were awake. LPA provided printed PIN 20-24-CCP-SP and PIN 19-02-CCP for review. Licensee and co-licensee will review PINs and will submit a written declaration stating what they learned from both PINs by POC due date. LPA also provided a copy of LIC 855.
Based on observation, the licensee did not comply with the section cited above in LPA observed (3) infants were missing proof of the 15-minute sleep log which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will submit proof of the 15-minute sleep log to LPA by POC due date. Licensee understands that the 15-minute sleep log is for all infants 0-24 months.
Based on observation, the licensee did not comply with the section cited above in LPA observed co-licensee's Pediatric CPR/FA expired on 01/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee. co-licensee has a scheduled Pediatric CPR/FA on 03/08. Proof of completion will be submitted to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in LPA observed (2) infants napping in their play pens with blankets. (1) infant was awake and also had a blanket in their play pen which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee and co-licensee immediately removed all blankets. LPA provided printed PIN 20-24-CCP-SP and PIN 19-02-CCP for review. Licensee and co-licensee will review PINs and will submit a written declaration stating what they learned from both PINs by POC due date. LPA also provided a copy of LIC 855.
Based on observation and record review, the licensee did not comply with the section cited above in that Assistan's CPR is not the required Pediatric CPR which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will have assistant take the correct CPR and submit proof of certificate to LPA Lopez by POC due date of 3/1/2023.
