Based on record review, staff 3 (S3) did not possess a compelte LIC503 Health Screening which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the director (S1) did not possess proof of immunization against measles which poses a potential health, safety or personal rights risk to persons in care.
On 12/03/24, licensee failed to maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for Faucet “A” – Sink Faucet, in the kitchen, with a result of 8.7 ppb. Water testing results identified with Action Level Exceedance as defined in WD section 101700.3. are not deemed safe to drink. This is a potential health and safety risk to children in care.
(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information:
(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.
(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:
(d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.
(d) Disaster drills shall be conducted at least every six months.
