Based on a file review performed by the LPA on 09/17/25, the licensee did not comply with the section cited above in 101217(d). The LPA observed that Staff S3, S4, and S5 were missing personnel files. The center director informed the LPA that staff S3-S5 have the required documents to work at this facility, however, their documents were stored in an off site location and not accessible at this time. This poses/posed a potential health, safety or personal rights risk to persons in care.
