Based on observation and interview, the facility had 14 children in care and the 13th and 14th did not meet the criteria in health and safety code which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would modify the facility's current attendance plan to bring the maximum number of children to 12 until children who meet the criteria for capacity of 13 or 14 are enrolled at the facility and would send the updated plan to LPA detailing how the requirements would be met.
Based on record review, Staff 3 (S3) did not possess proof of immunization against measles or pertussis which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would obtain a copy of S3's immunization record and send it to LPA by email at robert.maciel@dss.ca.gov
