Based on observation and record review, the licensee did not comply with the section cited above in missing sleep logs for C1 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will document their 15-minute sleep checks for one month and send proof of the logs to LPA.
Based on observation, the licensee did not comply with the section cited above, Licensee had a boppie pillow and blankets inside the 2 play pens present in the child care room, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written statement to the department after reviewing the regulation for Infant Safe Sleep and state that Licensee will ensure to keep the cribs/play pens free of any loose articles or objects. Licensee agrees to submit the statement by the POC due date. Licensee removed all the items during the annual visit.
Based on observation, the licensee did not comply with the section cited above in placing the inflated pool in the backyard upright against a wall. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA helped Licensee to place the inflated pool upright against a wall during the Visit. Licensee agrees to submit a written statement stating that Licensee will ensure that the inflated Pool or any kind of Wading pools will be emptied and stored upright immediately after use by the POC due date.

Resolution: Licensee agrees to create a sleep log for C1 and submit a completed sleep log to the department by the POC due date.
Based on observation and record review, the licensee did not comply with the section cited above in renewing her Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to renew her Mandated reporter Training and submit a copy of the certificate to the Department by the POC due date.
