Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing a current roster of children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the roster and send proof to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in missing tuberculosis testing for S3 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: S3 will obtain tuberculosis testing and licensee will send proof to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing mandated reporter training for S2 and S3 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure S2 and S3 complete their mandated reporter training and will send proof to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing the in person portion of the pediatric cpr and first aid training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will fully renew their pediatric cpr/first certification that will include the in-person training and will send proof to LPA.

Based on record review, the licensee did not comply with the section cited above as licensee and assistant had expired training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated the training would be updated and proof provided to LPA.

Based on record review, the licensee did not comply with the section cited above as (Ref#5) did not have immunizations, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would discuss with parents and either provide a schedule signed by the doctor for Ref#5 to begin and get current with immunizations or the child would be disenrolled. Licensee to provide proof to LPA.

Based on interview and record review, the licensee did not comply with the section cited above as one child (Ref#5) was missing documentation of 15 minute sleep checks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee stated she would begin documenting sleep checks for children under 2 years old and provide proof to LPA.

Based on record review, the licensee did not comply with the section cited above in completing LIC 9227 for C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete LIC 9227 for C2 and submit the a copy of the completed form to the Department by the POC due date.

Based on record review, the licensee did not comply with the section cited above Licensee did not have a sleep log available for C1 and C2 on file for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a sleep log for the infants in care and submit the completed log to the department by the POC due date.
