Based on observation and interview, the licensee did not comply with the section cited above in that the stairs are barricated with baby gate that is not in working conditions which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will purchase a new baby gate and install it by the stairs. Licensee will send photos confirming a new baby gate has been installed to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/26/2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that two of two infants did not have current 15-min. sleep logs for the past two weeks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will document 15-min. sleep logs starting tomorrow. Licensee will send photos of one week worth of documentation to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/26/2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that S2 did not have immunizations MMR and Tdap which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she ask S2 to obtain immunizations. Licensee will send photos confirming that S2 has obtain immunizations MMR and Tdap to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/26/2026.

Based on record review, the licensee did not comply with the section cited above S3 did not have TB record on file, which poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of S3 TB results by COB 02/28/2025

Based on record review, the licensee did not comply with the section cited above C6 did not have a current up to date sleeping log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin and document sleep logs for all infants in care. Proof of a months long sleeping log will be recorded and proof will be given to LPA Pulido by COB 03/18/2025

Resolution: Licensee stated she will get documentation on file and provide proof to LPA.

Based on record review, the licensee did not comply with the section cited above as licensee and assistant (Ref#1) had expired training, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in keeping the play pens and cribs free of all loose articles and objects for 3 out of 3 infants cribs/playpens which poses a potential health, safety or personal rights risk to persons in care. There was an infant sleeping in a crib with a blanket on and there were 2 infants inside their crib with blanket and toys.

Resolution: Licensee agrees to review Infant Safe Sleep Regulation and submit a statement to the department by the POC due date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 1 infant under 12 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete LIC 9227 and submit to the department by teh POC due date.
