Based on observation and interview, the licensee did not comply with the section cited above in that the stairs are barricated with baby gate that is not in working conditions which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will purchase a new baby gate and install it by the stairs. Licensee will send photos confirming a new baby gate has been installed to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/26/2026.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that two of two infants did not have current 15-min. sleep logs for the past two weeks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will document 15-min. sleep logs starting tomorrow. Licensee will send photos of one week worth of documentation to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/26/2026.

Resolution: Licensee agrees to review Infant Safe Sleep Regulation and submit a statement to the department by the POC due date.
