Based on observation the licensee did not comply with the section cited above where an infant was sleeping in a crib with a blanket covering them. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will conduct in-service training with all staff regarding infant safe sleep, and the cited regulation and provide proof of such to LPA by POC date.
Based on record review the licensee did not comply with the section cited above where the licensee had an expired Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that they will complete the Mandated Reporter training and provide proof of such to LPA by POC date.
Based on observation the licensee did not comply with the section cited above where LPA observed a roach and several flies within the facility. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to hire an exterminator, and provide proof of service to LPA by POC date.

Resolution: Licensee agrees not to use exersaucers, bouncers, walkers and any such items. Licensee removed the exersaucer and bouncer during the visit.
Based on record review, the licensee did not comply with the section cited above in having LIC 9227 completed for the infant in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the LIC 9227 and submit a copy to the department by the POC due date.
