Based on record review, the licensee did not comply with the section cited above in missing LIC 627 for C4 and C5 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete LIC 627 for both children and send a copy to LPA.

Based on interview and record review, the licensee did not comply with the section cited above as the licensee did not have a current roster on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will update their roster, date it, and send a copy to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in that 2 of 2 staff members (S1 and S2) did not have mandated reporter training completed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Assistant will complete the mandated reporter training and email certificates of completion to LPA.

Based on observation, the licensee did not comply with the section cited above in that Licensee had magentic locks disabled on the cabinets under the sink and in the ktichen making it accessible to children in care.The cabinets contained cleaning supplies, matches, and lighter which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA letter with plan to ensure the magnetic locks are enabled to ensure children do not have access to any hazardous items.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have documentation of the 15 minute check for the one infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA one weeks worth of documentation of the 15 minute sleep log for the infant in care.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have a personnel record for S2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email me completed personnel file for Assistant (S2).

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have any record of any emergency drills which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete emergency drill with children in care and email LPA copy of the emergency drill log to ensure it has been completed.
