Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having conducted a disaster drill every six months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete a disaster drill and send a copy of documented drill to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing file for C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure the obtain completed licensing forms from the parent/guardian of C1 and will send a completed file to LPA.

Based on observation and record review, the licensee did not comply with the section cited above in missing immunizations for C1 and missing MMR for C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will uptain immunization records for C1 and updated immunization records for C2 and send proof to LPA.

Based on observation and record review, the licensee did not comply with the section cited above in having an expired pediatric cardiopulmonary resuscitation (CPR) and pediatric first aid which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain pediatric CPR and first aid from the American Red Cross, American Heart Association, or an EMSA approved provider and will send proof of completion to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing sleep logs for C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete sleep logs for one month and send proof to LPA. Licensee will ensure sleep logs include the dates, times, position of child, and any signs of distress, including but not limited to labored breathing, flushed skin, and high body temperature.

Based on observation, the licensee did not comply with the section cited above as several hazardous items were accessible, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee made all the items inaccessible while LPA was still present.
