Based on observation, interview, and record review, the licensee did not comply with the section cited above in completing teh Health & Safety training as advised during the visit conducted on 11/07/2023 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was advised to complete the Health & Safety Training and submit the certificate to the department by the POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in ensuring all required LIC forms for C1, C2 and C3 were present on file for the department to review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to obtain completed LIC forms for C1, C2, and C3 and submit to the department by the POC due date.

Based on observation &interview, the licensee did not comply with the section cited above in backyard the trampoline had broken mesh hanging around it, the drawer to the left of the stove had sharp objects such as knives, can opener. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will fix the drawer latch and send LPA a photo of the drawer latch fixed and Licensee will fix the mesh around the trampoline and send a photo to LPA via email by 11/26/22
