Based on interview and record review, the licensee did not comply with the section cited above in checking on the napping infants every 15 mins and keeping a log which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the sleep log for C2 and C4 and submit a copy of the record to the department by the POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in obtaining a clearance for the young Adult child who resides at the facility which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the LIVE SCAN procedure immediately for the young Adult and submit to the department by the POC due date. Licensee was provided a LIVE SCAN Form.

Based on observation and interview, the licensee did not comply with the section cited above in keeping all cleaning compounds and other products out of reach of children which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to place child safe locks on the kitchen cabinet located under the sink and the drawers in the childcare restroom and submit pictures of the correction to the department by the POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in renewing the mandated reporter training which expired in March of 2024, S1 also did not have Mandated Reporter which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit the Mandated Reporter training certificate for themselves and S1 to the department by the POC due date.
